Below you can find a step-by-step guide explaining how to register as a new member or renew your existing membership at Living Well. Links to all the necessary forms have been included to expedite the registration process.

Prospective New Members:

  • Print and complete the New Member Form (under Quick Links).
  • Turn your completed form in to one of our staff members at the Living Well front desk who will then explain your payment options.
  • You may also be required to obtain a physician referral prior to your first work out at Living Well. To expedite this process, please provide the physician referral form (part of New Member Form) to your primary care physician and ask them to fax it to 352-846-3247.

Renewing Members:

  • Print and complete the Renewal Form (under Quick Links).
  • Turn your completed form in to one of our staff members at the Living Well front desk who will then explain your payment options.
  • You may also be required to obtain a physician referral prior to continuing to work out at Living Well. To expedite this process, please provide the physician referral form (part of Member Renewal Form) to your primary care physician and ask them to fax it to 352-846-3247.

Payment Options and Pricing:

  • Payment by check is preferred. All checks should be made payable to “University of Florida” – NOT Living Well.
  • Payroll deduction can be used by employees that are on 9-month or 12-month contracts with the University of Florida and purchase an annual (12-month) membership with Living Well.
    • 9-month employees
      • ONLY eligible for payroll deduction between July 1 and October 1 of each year
      • Deductions will be taken 16 times at $19.92 per pay period
    • 12-month employees
      • Eligible for payroll deduction year round
      • Deductions will be taken year round at $12.26 per pay period
  • All taxes and fees are included in prices listed in the payment chart.
